ST. LUKE HEALTH SERVICES IS LOCATED IN OSWEGO, NEW YORK, OVERLOOKING THE SCENIC OSWEGO RIVER. ST. LUKE IS EASILY ACCESSIBLE AND CLOSE TO NEIGHBORS AND FRIENDS IN THE GREATER-OSWEGO, NEW YORK COMMUNITY FOR OVER 30 YEARS.OUR SHORT-STAY REHAB UNITS OFFER INTENSIVE PHYSICAL, OCCUPATIONAL, OR SPEECH THERAPY LASTING GENERALLY NO MORE THAN 90 DAYS FOR THOSE WHO ARE RECOVERING FROM SURGERY, OR A MAJOR MEDICAL EVENT SUCH AS A STROKE OR HEART ATTACK. ST. LUKE ADULT DAY HEALTH CARE IS A DAILY HEALTH MONITORING AND ACTIVITIES PROGRAM DESIGNED TO PROMOTE INDEPENDENCE AND DELAY INSTITUTIONALIZATION OF THE DISABLED ADULT, AVAILABLE FOR 20 REGISTRANTS THROUGHOUT OSWEGO COUNTY. A MEDICAID ELIGIBLE PROGRAM, ADULT DAY HEALTH CARE IS A MEDICAL MODEL PROGRAM PROVIDING HEALTH AND MEDICAL CARE, INCLUDING CASE MANAGEMENT, HEALTH ASSESSMENT, PHYSICAL REHABILITATION, OCCUPATIONAL THERAPY, SPEECH THERAPY, DENTAL SERVICES, THERAPEUTIC DIETS, NUTRITIONAL EDUCATION AND MORE.
