ADULT RESIDENTIAL SERVICES PROGRAM: THIS PROGRAM ADDRESSES THE SPECIFIC FUNCTIONAL AND BEHAVIORAL NEEDS OF RESIDENTS WHICH PREVENT THEM FROM ACCESSING GENERIC HOUSING. 23,949 UNITS OF SERVICE PROVIDED.
BEHAVIORAL HEALTH AND WELLNESS CLINIC: ARTICLE 31 CLINIC TO PROVIDE ASSISTANCE TO INDIVIDUALS IN LEWIS COUNTY. BHWC PROVIDES COMPREHENSIVE MENTAL HEALTH SERVICES TO INDIVIDUALS WHO HAVE MENTAL HEALTH CONCERNS, BEHAVIOR PROBLEMS, EMOTIONAL DISTRESS, AND CO-OCCURRING DISORDERS. IT IS A NYS OFFICE OF MENTAL HEALTH LICENSED OUTPATIENT CLINIC. CLIENTS ARE SCREENED, EVALUATED AND TREATED BASED ON THEIR INDIVIDUAL NEEDS. SERVICES PROVIDED AT THE BHWC INCLUDE PSYCHIATRIC EVALUATION/ASSESSMENTS AND MEDICATION MANAGEMENT, GROUP THERAPY FOR ADULTS AND CHILDREN, INDIVIDUAL THERAPY FOR ADULTS AND CHILDREN, COUPLES COUNSELING, FAMILY THERAPY, PSYCHO EDUCATIONAL SESSIONS AND SUPPORT GROUPS. 12,866 VISITS PROVIDED.
CASE MANAGEMENT: HEALTH HOME CARE MANAGEMENT PROGRAM PROVIDES COORDINATED, COMPREHENSIVE MEDICAL AND BEHAVIORAL HEALTH CARE TO MEDICAID-ENROLLED ADULTS WITH CHRONIC CONDITIONS THROUGH CARE MANAGEMENT AND INTEGRATION THAT ASSURES ACCESS TO APPROPRIATE SERVICES, IMPROVES HEALTH OUTCOMES, REDUCES PREVENTABLE HOSPITALIZATIONS AND EMERGENCY ROOM VISITS, PROMOTES USE OF HEALTH INFORMATION TECHNOLOGY (HIT), AND AVOIDS UNNECESSARY CARE. 3,443 BILLABLE SERVICES PROVIDED.
