HUMAN SERVICE PLANNING (HSP) PROVIDES COLLABORATIVE PLANNING, CAPACITY BUILDING, TECHNICAL ASSISTANCE AND LEADERSHIP TRAINING TO NOT-FOR-PROFIT ORGANIZATIONS IN AN EFFORT TO CREATE WELL-ORGANIZED HEALTH AND HUMAN SERVICES DELIVERY SYSTEMS. HSC SERVES AS THE LEAD AGENCY FOR LOCAL HUD CONTINUUM OF CARE (COC) NY-510, A NETWORK OF SERVICE PROVIDERS AND INDIVIDUALS WHO WORK TOGETHER TO ELIMINATE HOMELESSNESS.
HSC'S 2-1-1 INFORMATION AND REFERRAL CONTACT CENTER SUPPORTS INDIVIDUALS AND ORGANIZATIONS BY PROVIDING COMPREHENSIVE, UP-TO-DATE INFORMATION FOR AN ARRAY OF COMMUNITY PROGRAMS AND SERVICES. THE CONTACT CENTER: SCREENS AND SCHEDULES LOW-INCOME TAX PREPARATION CLIENTS FOR SELECT REGIONAL VITA PROGRAMS; SCREENS AND SCHEDULES FOR FOOD PANTRY DELIVERIES; PARTICIPATES IN COUNTY-WIDE TRANSPORTATION PLANNING EFFORTS; AND SUPPORTS COORDINATED, UNDUPLICATED COMMUNITY INITIATIVES BY PROVIDING UP-TO-DATE PROGRAM AND SERVICE INFORMATION.
THE HEALTH PLANNING COUNCIL (HPC) IS COMMITTED TO IMPROVING COMMUNITY HEALTH THROUGH HEALTH PLANNING AND PROMOTION, AND OPERATING PROGRAMS TO INCREASE ACCESS TO HEALTH CARE.
