FOSTER CARE: PROVIDES BOTH LONG AND SHORT-TERM FOSTER CARE SERVICES FOR CHILDREN UNABLE TO BE CARED FOR BY THEIR BIRTH FAMILIES OR LEGAL GUARDIANS. WHILE RECEIVING SUPPORTIVE CARE, COUNSELING AND SUPERVISION, THE CHILD IN CARE IS ABLE TO LIVE WITHIN A FAMILY HOME, ATTEND PUBLIC SCHOOL, AND BE AN ACTIVE MEMBER OF THEIR COMMUNITY. WHILE ENSURING SAFETY, THE GOAL OF FOSTER CARE IS TO ASSIST THE YOUTH ACHIEVE PERMANENCY.

CASE MANAGEMENT: CARE COORDINATION OF NORTHERN NEW YORK PROVIDES CRITICAL SERVICES AND CARE MANAGEMENT TO SERIOUSLY EMOTIONALLY DISTURBED CHILDREN AND ADULTS LIVING IN THE COMMUNITY. SERVICES INCLUDE ASSESSMENT AND TRIAGE OF NEEDS, REFERRALS TO OTHER PROVIDERS, AND ASSISTING INDIVIDUALS IN NAVIGATING THE COMPLEX HEALTH CARE SYSTEM.


COMMUNITY CLINIC OF JEFFERSON COUNTY: AN ARTICLE 31 BEHAVIORAL AND MENTAL HEALTH OUT-PATIENT CLINIC TREATING A MAJORITY OF MEDICAID PATIENTS INCLUDING ADULTS AND CHILDREN. SERVICES INCLUDE INDIVIDUAL, COUPLES, FAMILY AND GROUP THERAPY, PSYCHOPHARMACOLOGIC TREATMENT, FORENSIC EVALUATIONS AND TREATMENT, NEUROFEEDBACK AND ASSESSMENTS. CHILDREN AND FAMILY TREATMENT SUPPORT SERVICES (CFTSS) ARE PROVIDED TO INDIVIDUALS THROUGHOUT THE COMMUNITY.
