TO PROMOTE INDEPENDENT LIVING FOR PEOPLE WITH DISABILITIES THROUGH WIDE VARIETY OF SERVICES, INFORMATION, EDUCATION OUTREACH AND TRAININGS TO PEOPLE WITH DISABILITIES.


SDMC SURROGATE DECISION MAKING PROGRAM - IS AN ALTERNATIVE TO THE COURT SYSTEM FOR INDIVIDUALS WHO LACK CAPACITY TO PROVIDE INFORMED CONSENT, DO NOT HAVE A LEGALLY AUTHORIZED DECISION MAKER TO ACT ON THEIR BEHALF AND ARE IN NEED OF A NON-EMERGENCY MAJOR MEDICAL PROCEDURES. SDMC ALSO HANDLES END-OF-LIFE DECISION MAKING FOR QUALIFYING INDIVIDUALS.


CARES - CDCI BOARD OF DIRECTORS AND EXECUTIVE STAFF MADE PRUDENT, REASONABLE DECISIONS WITH REGARDS TO THE USE OF THE CIL CARES ACT FUNDING. TO THAT END, DURING THIS CONTRACT YEAR CDCI USED THE CARES FUNDING TO PURCHASE TECHNOLOGY, WHICH INCLUDED LAPTOPS, PRINTERS, SCANNERS, CELLPHONES, AND APPROPRIATE SOFTWARE TO ENABLE AND SUPPORT STAFF IN PROVIDING REMOTE SERVICE DELIVERY TO OUR CONSUMERS. THE TECHNOLOGY PURCHASED ALSO PROVIDED CDCI STAFF THE ABILITY TO ATTEND TRAININGS AND STAY CONNECTED THROUGH ZOOM MEETINGS WITH OTHER COMMUNITY-BASED ORGANIZATIONS.
