THE COMMUNITY HOSPICE, FORMED IN 1997, IS LOCATED THROUGHOUT EIGHT COUNTIES IN EASTERN NEW YORK STATE. WHEN PEOPLE ARE IN THE FINAL STAGES OF LIFE, THE COMMUNITY HOSPICE HELPS THEM TO LIVE WITH DIGNITY, WITHOUT PAIN, AND IN FAMILIAR SURROUNDINGS.THE COMMUNITY HOSPICE PROVIDES COMPREHENSIVE COMFORT-ORIENTED SERVICES AND COMPASSIONATE ATTENTION IN PATIENTS' HOMES, IN THE HOSPITAL, AT NURSING HOMES, OR A HOSPICE RESIDENCE. A PERSON IS ELIGIBLE FOR HOSPICE CARE IF THEIR PHYSICIAN ESTIMATES A LIFE EXPECTANCY OF SIX MONTHS OR LESS. PATIENTS CAN EXPECT REGULAR VISITS FROM A CARING TEAM OF PROFESSIONALS THAT INCLUDES NURSES, SOCIAL WORKERS, CHAPLAINS, AND HOME HEALTH AIDES. A PLAN OF CARE IS DEVELOPED FOR EACH INDIVIDUAL BASED ON THE PARTICULAR NEEDS OF THE PATIENT AND FAMILY.
