NUTRITION, PHYSICAL ACTIVITY AND OBESITY: CHRONIC DISEASE IS A MAJOR CAUSE OF DISABILITY AND DEATH IN NEW JERSEY. SEVEN OF THE TEN LEADING CAUSES OF DEATH IN NEW JERSEY ARE CHRONIC DISEASES. TOBACCO, PHYSICAL INACTIVITY, OVERWEIGHT/OBESITY ARE THREE OF THE UNDERLYING PREVENTABLE CAUSES OF DEATH. DEVELOPING A FRAMEWORK FOR ADVANCING HEALTH AND WELLNESS PROGRAMS TO IMPACT COMMUNITIES THROUGH POLICY, SYSTEM AND ENVIRONMENTAL CHANGE HAS REAL POTENTIAL TO IMPACT FAMILIES AND INDIVIDUALS IN THEIR COMMUNITIES.
SUPPLEMENTAL NUTRITIONAL ASSISTANCE PROGRAM - EDUCATION: THE GOAL OF SNAP-ED IS TO IMPROVE THE LIKELIHOOD THAT PERSONS ELIGIBLE FOR SNAP WILL MAKE HEALTHY FOOD CHOICES WITHIN A LIMITED BUDGET AND CHOOSE A PHYSICALLY ACTIVE LIFESTYLE CONSISTENT WITH THE CURRENT DIETARY GUIDELINES FOR AMERICANS AND MYPLATE. THE FOCUS IS TO IMPLEMENT STRATEGIES OR INTERVENTIONS TO HELP THE SNAP-ED TARGET AUDIENCE ESTABLISH HEALTHY EATING HABITS AND ACTIVE LIEFSTYLE. ALSO, PRIMARY PREVENTION OF DISEASE AMONG THE TARGET AUDIENCE THAT EXHIBIT A HIGHER RISK FOR NUTRITION-RELATED CHRONIC DISEASES.
PRA/SPECT: THE NEW JERSEY DEPARTMENT OF HEALTH SUPPORTS EVIDENCED BASED PROGRAMS TO IMPROVE THE HEALTH AND WELL BEING OF PREGNANT WOMEN AND YOUNG CHILDREN IN ALL COUNTIES IN THE STATE. A CORE COMPONENT OF IDENTIFYING THE NEEDS OF FRAGILE FAMILIES IS ACCESS TO THE BEST PROGRAM FOR EACH FAMILY BASED ON THE FAMILY'S NEEDS AND THE COMMUNITY'S RESOURCES.
