TO PROVIDE MANAGEMENT SUPPORT AND CONSULTATION SERVICES TO HELP THE MATERNAL CHILD HEALTH CONSORTIUM AND OTHER COMMUNITY BASED ORGANIZATIONS STREAMLINE.

PRIMARY & RURAL HEALTH: FHI IS APPLYING THE COMMUNITY-BASED STEWARDSHIP APPROACH TO IMPROVE RURAL HEALTH, TOBACCO USE AND ADDICTION IN RURAL COMMUNITIES, TO FOSTER ACCESS TO HEALTH CARE SERVICES AND RESOURCES AMONG THE RESIDENTS OF CAPE MAY AND SALEM COUNTIES. FHI HAS SELECTED VOLUNTEERS IN MEDICINE AND THE PUERTO RICAN ACTION COMMITTEE OF SOUTHERN NEW JERSEY, SPECIFICALLY BECAUSE OF THEIR INCLUSION OF COMMUNITY HEALTH WORKERS (CHW) IN THEIR WORK. THIS MODEL TAKES THE SERVICES INTO THE COMMUNITIES, WHERE ACCESS TO CARE IS LIMITED AND DIFFICULT DUE TO A LACK OF HEALTH CARE PROGRAMS AND VERY LIMITED PUBLIC TRANSPORTATION. SUPPLEMENTAL NUTRITIONAL ASSISTANCE PROGRAM - EDUCATION: THE GOAL OF SNAP-ED IS TO IMPROVE THE LIKELIHOOD THAT PERSONS ELIGIBLE FOR SNAP WILL MAKE HEALTHY FOOD CHOICES WITHIN A LIMITED BUDGET AND CHOOSE A PHYSICALLY ACTIVE LIFESTYLE CONSISTENT WITH THE CURRENT DIETARY GUIDELINES FOR AMERICANS AND MYPLATE. THE FOCUS IS TO IMPLEMENT STRATEGIES OR INTERVENTIONS TO HELP THE SNAP-ED TARGET AUDIENCE ESTABLISH HEALTHY EATING HABITS AND ACTIVE LIEFSTYLE. ALSO, PRIMARY PREVENTION OF DISEASE AMONG THE TARGET AUDIENCE THAT EXHIBIT A HIGHER RISK FOR NUTRITION-RELATED CHRONIC DISEASES.


PRA/SPECT: THE NEW JERSEY DEPARTMENT OF HEALTH SUPPORTS EVIDENCED BASED PROGRAMS TO IMPROVE THE HEALTH AND WELL BEING OF PREGNANT WOMEN AND YOUNG CHILDREN IN ALL COUNTIES IN THE STATE. A CORE COMPONENT OF IDENTIFYING THE NEEDS OF FRAGILE FAMILIES IS ACCESS TO THE BEST PROGRAM FOR EACH FAMILY BASED ON THE FAMILY'S NEEDS AND THE COMMUNITY'S RESOURCES.
