SUPPORTED HOUSING: PROGRAM FOR CONSUMERS WITH PSYCHIATRIC DISABILITIES WHO ARE ABLE TO LIVE ON THEIR OWN WITH VERY LITTLE STAFF SUPPORT. STAFF VISIT CONSUMERS AT LEAST ONCE PER MONTH BUT MORE OFTEN PROVIDES SERVICES ON A MORE FREQUENT BASIS WHEN NEEDED.
APARTMENT TREATMENT: PROVIDES SERVICES TO 96 CONSUMERS WITH PSYCHIATRIC DISABILITIES IN THEIR AGENCY PROVIDED APARTMENTS. STAFF MEMBERS VISIT CONSUMERS ANYWHERE FROM ONCE A DAY TO ONCE A WEEK, DEPENDING ON THE NEEDS OF THE INDIVIDUAL.
COMMUNITY RESIDENCES: PROVIDES SUPPORT AND SERVICES TO CONSUMERS WITH PSYCHIATRIC DISABILITIES 24 HOURS A DAY 7 DAYS A WEEK IN A GROUP HOME SETTING. AGENCY OWNS 4 HOMES, 3 CONTAINING 10 CONSUMERS, 1 CONTAINING 11 CONSUMERS, AND ROCKVILLE CENTRE SPECIALIZING IN YOUNG ADULTS AGES 18-30.
SHELTER PLUS CARE:IS FOR CONSUMERS WITH PSYCHIATRIC DISABILITIES WHO ARE HOMELESS PRIOR TO ADMISSION. SAIL PROVIDES SERVICES TO INDIVIDUALS TO HELP MAINTAIN THEIR HOUSING.ASSERTIVE COMMUNITY TREATMENT:A CLINICAL PROGRAM SERVING 68 CONSUMERS BY A PROFESSIONAL STAFF THAT SEE PEOPLE IN THEIR NATURAL ENVIRONMENT WHO ARE UNABLE AND UNWILLING TO UTILIZE THE TRADITIONAL MENTAL HEALTH SYSTEM.ASSERTIVE COMMUNITY TREATMENT PROGRAM (ACT):THIS IS A NON-TRADITIONAL MENTAL HEALTH PROGRAM. THE ACT TEAM SERVES "HIGH NEED" PERSONS WHO HAVE BEEN UNWILLING OR UNABLE TO UTILIZE MAINSTREAM MENTAL HEALTH SERVICES. THE PROFESSIONAL STAFF PROVIDES SERVICES TO CONSUMERS WHO "THE SYSTEM" BELIEVES ARE THE MOST DIFFICULT. THE PROGRAM IS FUNDED THROUGH A GRANT WITH THE NASSAU COUNTY OFFICE OF MENTAL HEALTH, CHEMICAL DEPENDENCY AND DEVELOPMENT DISABILITIES AND MEDICAID.
