OUR MISSION IS TO INCREASE HEALTHY BEHAVIORS BY RESIDENTS OF NORTHERN NEW YORK STATE IN ORDER TO DECREASE THE INCIDENCE OF CARDIAC DISEASE, STROKE, AND OTHER CHRONIC DISEASES AND THEIR RISK FACTORS IN THE REGION. THE MISSION STATEMENT SHARED IN PRINTED MATERIALS TODAY SAY THAT "OUR MISSION IS TO DESIGN, DEVELOP, AND IMPLEMENT STRATEGIES TO DECREASE THE INCIDENCE OF CARDIAC DISEASE, STROKE, AND RELATED CHRONIC ILLNESS IN NORTHERN NEW YORK STATE."

WE CONTINUED OUR EFFORTS TO BUILD A CHRONIC DISEASE PREVENTION SYSTEM THAT HAS THE CAPACITY TO SUPPORT REGULAR, ONGOING DELIVERY OF EVIDENCE-BASED CHRONIC DISEASE PREVENTION PROGRAMS ACROSS SEVEN COUNTIES IN RURAL, UPSTATE NEW YORK. WE ALSO CONTINUED IMPLEMENTATION OF OUR TOBACCO CESSATION AND CHILDHOOD OBESITY PREVENTION PROGRAMS. OF NOTE, WE ENGAGED WITH 29 NORTH COUNTRY HEALTHCARE PROVIDER ORGANIZATIONS TO FACILITATE ESTABLISHMENT AND/OR IMPROVEMENT OF THEIR NICOTINE CESSATION TREATMENT PROTOCOLS; SUPPORTED 7 RURAL SCHOOL DISTRICTS AND 6 EARLY CHILDHOOD EDUCATION PROGRAMS TO IMPLEMENT POLICIES, PLANS, AND PROGRAMS DESIGNED TO INCREASE YOUTH ACCESS TO NUTRITIOUS FOODS AND/OR OPPORTUNITIES FOR RECREATIONAL ACTIVITIES; HELPED 4 COMMUNITY SITES TO IMPROVE THEIR ABILITY TO OFFER NUTRITIOUS FOOD OPTIONS TO THE PEOPLE THEY SERVE; ASSISTED 6 MUNICIPALITIES WITH EFFORTS TO MAKE THEIR COMMUNITIES MORE PEDESTRIAN FRIENDLY; MAINTAINED AND PROMOTED A WEBSITE THAT FACILITATES ENROLLMENT IN EVIDENCE-BASED CHRONIC DISEASE PREVENTION AND SELF-MANAGEMENT EDUCATION (CDPSME) PROGRAMS; AND LED EFFORTS OF A NETWORK OF 35+ HEALTH AND SOCIAL CARE ORGANIZATIONS ACROSS A 7 COUNTY REGION TO IMPLEMENT OUR REVENUE DEVELOPMENT PLAN; MARKETING AND COMMUNICATIONS PLAN; AND CONDUCT A SEARCH FOR A PROGRAM SCHEDULING AND PARTICIPANT TRACKING PLATFORM.
