THE MISSION OF PFTL IS TO PROVIDE, ON A NOT-FOR-PROFIT BASIS, TRANSITIONAL LIVING FACILITIES AND LONG TERM, PERMANENT, SUPPORTED HOUSING FOR PERSONS WITH CHRONIC PSYCHIATRIC ILLNESSES, TO PROMOTE GREATER PUBLIC UNDERSTANDING OF MENTAL ILLNESS AND TO PROVIDE EDUCATIONAL OPPORTUNITIES FOR MENTAL HEALTH PRACTITIONERS.

THE ORGANIZATION OFFERS THE FOLLOWING PROGRAM SERVICES:1) RESIDENTIAL LIVING CENTER - THE GROUP HOME CONSISTS OF A FIFTEEN BED STATE LICENSED FACILITY, WHICH IS LOCATED IN THE ORGANIZATION'S MAIN BUILDING. RESIDENTS RECEIVE TWENTY-FOUR HOUR STAFF SUPERVISION, MEDICATION MONITORING AND CASE MANAGEMENT.2) SUPERVISED APARTMENT - THE APARTMENTS ARE A NON-LICENSED FACILITY ALSO LOCATED IN THE ORGANIZATION'S MAIN BUILDING, WHICH HAS SPACE FOR UP TO FIFTEEN RESIDENTS. THE GOAL OF THE APARTMENT PROGRAM IS TO EMPOWER THE RESIDENTS AND ENCOURAGE SKILL BUILDING IN AN INDEPENDENT LIVING ENVIRONMENT SUCH AS COOKING AND CLEANING. RESIDENTS IN THIS RESIDENCE ARE ENCOURAGED TO TAKE A MAJOR ROLE IN PLANNING, ORGANIZING AND LEADING ACTIVITIES.3) INDEPENDENT CARE PROGRAM - UPON DISCHARGE FROM THE GROUP HOME OR APARTMENTS, A FORMER RESIDENT MAY STILL RECEIVE CASE MANAGEMENT AND OTHER SUPPORT SERVICES WHILE RESIDING OUTSIDE THE FACILITY.
