THE CAREALIGN STUDY'S MAIN OBJECTIVE IS TO BUILD, TEST, AND HEIGHTEN AWARENESS OF, AND CREATE DEMAND FOR, PATIENT HEALTH OUTCOME GOALS AND CARE PREFERENCE-DIRECTED CARE (I.E. PATIENT GOALS-DIRECTED CARE) FOR OLDER ADULTS WITH MULTIPLE CHRONIC CONDITIONS ACHIEVED THROUGH PRIMARY/SPECIALTY CARE ALIGNMENT. 1.
