WE HELP MORE THAN 5,000 ADULTS, CHILDREN, AND FAMILIES AT RISK OF OR EXPERIENCING HOMELESSNESS.

HOUSING: OWNING AND OPERATING MORE THAN 30 PROPERTIES, HARBOR HOMES PROVIDES THE HOUSING AND SUPPORTS NEEDED TO ESCAPE HOMELESSNESS AND SUSTAIN INDEPENDENCE IN THE COMMUNITY. THE LARGEST BUILDER AND SUPPLIER OF SUPPORTIVE HOUSING FOR THOSE EXPERIENCING HOMELESSNESS IN NH, WE SERVE 1,000 INDIVIDUALS ANNUALLY THROUGH OUR OWN HOUSING AND THE DISTRIBUTION OF VOUCHERS USED IN THE PRIVATE RENTAL MARKET. OUR SPECIALIZED VETERANS FIRST PROGRAMMING USES OUTREACH, HOUSING, EMERGENCY SUBSIDIES, AND EMPLOYMENT SUPPORTS TO ENSURE NO VETERAN REMAINS HOMELESS IN NEW HAMPSHIRE.

WE PROVIDE HIGH QUALITY HOUSING AND RESIDENTIAL SERVICES, PRIMARY AND BEHAVIORAL HEALTH CARE, SUBSTANCE USE DISORDER TREATMENT, HOME CARE, HIV/AIDS CARE, VETERAN SERVICES, EMPLOYMENT SUPPORTS, AND SUPPORTS TO INDIVIDUALS AND FAMILIES WHO ARE EXPERIENCING OR AT RISK OF HOMELESSNESS. OUR UNIQUE MODEL ALLOWS US TO PROVIDE THE WIDE RANGE OF COLLABORATIVE SERVICES PEOPLE NEED TO GAIN AND SUSTAIN INDEPENDENCE. HARBOR CARE HEALTH AND WELLNESS CENTER (HCHWC): THE HCHWC IS S A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) THAT SERVES THOUSANDS OF INDIVIDUALS AND FAMILIES, INCLUDING VETERANS, WHO ARE EXPERIENCING OR AT RISK OF HOMELESSNESS, LOW INCOME, UNINSURED, AND/OR UNDERINSURED. HCHWC PROVIDES PRIMARY, PREVENTIVE, BEHAVIORAL, PHARMACY, HIV, SUBSTANCE MISUSE, AND DENTAL HEALTH CARE SERVICES, AND DOES NOT TURN ANYONE AWAY DUE TO AN INABILITY TO PAY FOR SERVICES. IN ADDITION, THE HCHWC INTEGRATES SERVICES THAT IMPACT SOCIAL DETERMINANTS OF HEALTH, SUCH AS HOUSING, EMPLOYMENT, EDUCATION AND TRAINING, HOME HEALTHCARE, AND SUPPORTS WITH SERIOUS CHRONIC HEALTH CONDITIONS.
PEER RECOVERY SUPPORT SERVICES: SINCE 2016, HARBOR CARE HAS SERVED AS THE FACILITATING ORGANIZATION FOR THE STATE OF NEW HAMPSHIRE'S EFFORTS TO SUPPORT THE SUSTAINABLE AND INTEGRATED DEVELOPMENT OF RECOVERY COMMUNITY ORGANIZATIONS (RCO) AND THEIR DELIVERY OF PEER RECOVERY SUPPORT SERVICES (PRSS). WE'VE HELPED 13 RECOVERY COMMUNITY ORGANIZATIONS OPERATING 20 RECOVERY COMMUNITY CENTERS GROW AND THRIVE, AS THEY SERVE THOUSANDS OF INDIVIDUALS IN ALL STAGES OF RECOVERY THROUGHOUT NEW HAMPSHIRE.
