TO IMPROVE THE HEALTH AND WELL BEING OF WESTERN NEW YORK RESIDENTS THROUGH AWARENESS, PREVENTION, WELLNESS, AND OTHER EDUCATION PROGRAMS FOCUSED ON COMMUNITY HEALTH PRIORITIES.
