FACILITATE SERVICES FOR INDIVIDUALS AND FAMILIES IN NEED, INCLDUING INFORMATION, EDUCATION AND GUIDANCE TO COORDINATE SERVICE ACCESS AND DELIVER IN HEALTH CARE, BEHAVIOR HEALTH AND OTHER HUMAN SERVICES IN PARTNERSHIP WITH THE LOCAL MEDICAL AND SOCIAL SERVICE COMMUNITY.
