WE PROVIDE PRIMARY MEDICAL CARE, WELL CHILD VISITS INCLUDING IMMUNIZATIONS, STD/HIV SCREENING AND TREATMENT, AND HIV PREVENTION SERVICES TO LOW-INCOME INDIVIDUALS WITH A SERVICE DELIVERY MODEL THAT ASSURES THE INTEGRATION OF ENABLING SERVICES INCLUDING CASE MANAGEMENT, OUTREACH, TRANSPORTATION, ON-SITE PHARMACY, AND ACCESS TO A CONTINUUM OF CARE AND SPECIAL CARE SERVICES.
