THE ARIZONA HEALTHIEST STATE FOUNDATION (AHSF) BRINGS ARIZONA LEADERS TOGETHER TO HELP EMPOWER ARIZONANS TO MAKE THEIR LIFE WISHES AND CARE DIRECTIVES KNOWN AND TO EQUIP THEIR HEALTHCARE TEAMS WITH RESOURCES TO HONOR THESE DECISIONS. DURING 2021:*AHSF CONVENED THE COALITION TO TRANSFORM SERIOUS ILLNESS CARE. A GROUP OF OVER 40 COMMUNITY LEADERS. THE COALITION DESIGNED A PROGRAM TO BRING HOME AND COMMUNITY BASED PALLIATIVE CARE TO COMMUNITIES A WROTE A REPORT OF THE ACTIVITIES THAT SERVES AS A SPRINGBOARD FOR ADDITIONAL FUNDING REQUESTS.*THE COALITION, SUPPORTED BY THE AHSF TAUGHT CLASSES FOR ADVANCE CARE PLANNING, CONVERSATIONS IN SERIOUS ILLNESS AND POLST TO A TOTAL OF 538 PEOPLE IN 41 CLASSES IN 2021.*AHSF SERVED AS THE STATE LEAD FOR POLST.*AHSF SERVED AS INVITED PRESENTERS AT 3 CONFERENCES IN 2021.
