SEE SCHEDULE O PATHWAYS PRIVATE DUTY IS A LICENSED HOME CARE ORGANIZATION, LICENSED THROUGH THE DEPARTMENT OF SOCIAL SERVICES, THAT ASSISTS OLDER ADULTS AND THE CHRONICALLY ILL IN THEIR ACTIVITIES OF DAILY LIFE. THE SERVICES ARE PROVIDED ON AN HOURLY BASIS TO INDIVIDUALS IN THEIR PLACE OF RESIDENCE UP TO 24 HOURS A DAY, ON A TEMPORARY OR LONG TERM BASIS. PATHWAYS PRIVATE DUTY ALSO PROVIDES HOURLY CARE MANAGEMENT SERVICES PERFORMED BY AND/OR SUPERVISED BY A LCSW, CERTIFIED IN GERIATRIC CARE MANAGEMENT TO ASSIST OLDER PEOPLE AND THEIR FAMILIES IN COORDINATING NEEDED SERVICES ON A HOLISTIC BASIS.

SEE SCHEDULE O PATHWAYS CONTINUOUS CARE IS A COMMUNITY BASED NONPROFIT ORGANIZATION WHICH PROVIDES OLDER ADULTS AND THE CHRONICALLY ILL COMPANIONSHIP, SUPERVISION AND ASSISTS WITH THEIR ACTIVITIES OF DAILY LIVING IN THEIR PLACE OF RESIDENCE, THUS ENABLING THEM TO REMAIN AT HOME SAFELY AND MAINTAIN THEIR INDEPENDENCE. CARE MANAGEMENT SERVICES ARE PROVIDED ON AN AS NEEDED BASIS, TO ENSURE THE NECESSARY SUPPORT TO MANAGE ALL SITUATIONS AS THEY ARISE. THE SERVICES ARE OVERSEEN BY A LICENSED CLINICAL SOCIAL WORKER. ATTENDANT SERVICES ARE PROVIDED ON A 3 TO 24 HOUR BASIS AS OUTLINED IN THEIR PERSONAL CARE PLAN WHICH IS CREATED WITH THEM AND THEIR FAMILY. SERVICES ARE PROVIDED BY HOME CARE AIDS, MANY OF WHICH HAVE OBTAINED STATE CERTIFICATION IN HOME HEALTH OR NURSING ASSISTANT PROGRAMS. ADDITIONALLY WE HAVE LICENSED NURSES AVAILABLE FOR QUESTIONS AND HELP WITH CARE PLANNING. THE HOME CARE AIDES PERFORM PERSONAL CARE SERVICES, INCLUDING BATHING, DRESSING, INCONTINENCE CARE, GROOMING, AMBULATION, MEAL PREP, MEDICATION REMINDERS AND HELP IN TRANSFERS. ADDITIONALLY, THEY PROVIDE HOME SUPPORT SERVICES INCLUDING COMPANIONSHIP, SOCIALIZATION, LIGHT HOUSEKEEPING, TRANSPORTATION AND SHOPPING. CAREGIVERS ARE SPECIALLY TRAINED IN SERVICES RELATED TO HOSPICE, HOSPICE RESPITE, ALZHEIMERS, DEMENTIA, PARKINSONS AND OTHERS.
