THE ORGANIZATION PROVIDED HOME AND INPATIENT HOSPICE CARE, PALLATIVE CARE VISITS, HOME HEALTH CARE, OUTPATIENT THERAPY VISITS, AND TRANSITIONAL CARE FOR THE CENTRAL OREGON REGION. THE ORGANIZATION SERVED OVER 1,132 HOSPICE PATIENTS AT THE ORGANIZATION'S IN-PATIENT FACILITY, PATIENT HOMES OR IN OTHER FACILITIES. THE ORGANIZATION'S MD'S AND RN'S PROVIDED 1,266 PALLATIVE CARE VISITS TO PATIENTS NOT ENROLLED IN EITHER HOSPICE OR HOME HEALTH. THE ORGANIZATION'S HOME HEALTH CAREGIVERS SERVED OVER 1,623 PATIENTS AND THE OUTPATIENT THERAPY PROGRAM PROVIDED 60 VISITS.
