LONG-TERM CARE HEALTH SERVICES ARE PROVIDED IN A 58 BED HEALTH CENTER WITHIN THE SUN VALLEY LODGE. THE NURSING PROGRAM IS LICENSED BY THE ARIZONA DEPARTMENT OF HEALTH SERVICES AS A LONG TERM CARE PROVIDER LICENSE NCI- 258.
WE OFFER INDEPENDENT LIVING IN OUR TWO BEDROOM GARDEN APARTMENTS THAT INCLUDES DAILY MEALS, EMERGENCY ACCESS TO OUR CAREGIVERS, AROUND THE CLOCK MAINTENANCE, AND ACCESS TO ALL OUR COMMUNITY AMENITIES AND FUNCTIONS. WE ALSO OFFER INDEPENDENT LIVING IN OUR INN LIVING STUDIO AND ONE BEDROOM APARTMENTS WHERE THEY WILL RECEIVE THREE MEALS A DAY AND WEEKLY HOUSEKEEPING.
ASSISTED LIVING PROGRAM SERVICES ARE PROVIDED TO RESIDENTS IN ONE OF THE 73 STUDIO OR ONE BEDROOM APARTMENTS. RESIDENTS CAN CHOOSE FROM A NUMBER OF SERVICES TO CREATE A PACKAGE OF CARE THAT BEST SUITS THEIR NEEDS. SUN VALLEY LODGE IS LICENSED FOR ASSISTED LIVING BY THE ARIZONA DEPARTMENT OF HEALTH SERVICES LICENSE AL4866C.
