VALLEY HEALTH PARTNERS (THE CORPORATION) IS A NOT-FOR-PROFIT CORPORATION WITH FACILITIES LOCATED IN ALLENTOWN, PENNSYLVANIA. THE CORPORATION WAS ESTABLISHED TO PROVIDE PRIMARY AND PREVENTATIVE HEALTH CARE SERVICES TO THE RESIDENTS OF ITS SERVICE AREA, WITH A PARTICULAR FOCUS ON LOW-INCOME, MEDICALLY UNDERSERVED INDIVIDUALS. EFFECTIVE SEPTEMBER 1, 2021, THE CORPORATION RECEIVED APPROVAL FROM THE U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES, HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) FOR FEDERALLY QUALIFIED HEALTH CENTER (FQHC) LOOK-ALIKE DESIGNATION THROUGH FEBRUARY 28, 2025. CONTINUED ON SCHEDULE O.ON FEBRUARY 22, 2022, THE CORPORATION RECEIVED THEIR APPROVED INTERIM REIMBURSEMENT RATES FOR THE MEDICAL ASSISTANCE PROGRAM.
