TELEIOS COLLABORATIVE NETWORK SUPPORTS ITS MEMBERS AND THEIR LOCAL COMMUNITY-BASED PROGRAMS FOR PALLIATIVE AND SUPPORTIVE CARE WHO AIM TO ADDRESS THE PHYSICAL, PSYCHOLOGICAL, AND SPIRITUAL NEEDS OF THE SERIOUSLY OR CHRONICALLY ILL AND THEIR FAMILY MEMBERS. TELEIOS COLLABORATIVE NETWORK ENABLES MEMBERS TO LEVERAGE BEST PRACTICES, ECONOMIES OF SCALE, AND COLLABORATE IN WAYS THAT BETTER PREPARE EACH ORGANIZATION TO PARTICIPATE IN EMERGING ALTERNATIVE PAYMENTS MODELS AND ADVANCE THEIR CHARITABLE MISSIONS OF PROMOTING HEALTH THROUGH HOSPICE, PALLIATIVE, AND ADVANCE ILLNESS CARE IN LOCAL COMMUNITIES.
