HUGH LANE WELLNESS FOUNDATION WORKS TO IMPROVE THE HEALTH OF ALL OF OUR LGBTQ+ AND HIV COMMUNITIES BY CREATING PROGRAMMING AND DELIVERING SERVICES THAT SUPPORT HEALTHY MINDS AND HEALTHY HEARTS. OUR WORK FOCUSES ON REMOVING SYSTEMIC BARRIERS THAT BLOCK LGBTQ+ INDIVIDUALS FROM ATTAINING HEALTH EQUITY. WE STRIVE TO REDUCE AND ELIMINATE HEALTH DISPARITIES BY PROVIDING DIRECT SERVICES, RESOURCES AND CREATING ENGAGING PROGRAMMING TO CONNECT WITH OTHER COMMUNITY MEMBERS. WHEN WE WORK TOGETHER TO ADDRESS SOCIAL DETERMINANTS OF HEALTH, ELIMINATE BARRIERS TO CARE AND SUPPORT RESILIENCY AND JOY FOR OUR LGBTQ+ COMMUNITIES, WE CAN BUILD HEALTHY MINDS AND HEALTHY HEARTS.
