HOMELESS AND HOMELESSNESS PREVENTION PROGRAMS-CCSOMO CASE MANAGERS WORK WITH HOUSEHOLDS WHO ARE HOMELESS OR AT-RISK OF HOMELESSNESS PROVIDING INTENSIVE CASE MANAGEMENT AND TEMPORARY FINANCIAL ASSISTANCE FOR RENT AND UTILITIES, WITH THE GOALS OF ACHIEVING PERMANENT HOUSING, STABILITY, AND FINANCIAL SELF-SUFFICIENCY. THE PROGRAM SERVES VETERANS, FAMILIES, SENIORS, THE DISABLED AND INDIVIDUALS REENTERING THE COMMUNITY BY ASSISTING THEM IN ADDRESSING THEIR BARRIERS TO STABLE HOUSING. IN THE PAST YEAR, CCSOMO PROVIDED SERVICES TO 2751 INDIVIDUALS IN 1289 HOUSEHOLDS.
TARGETED CASE MANAGEMENT IS PROVIDED BY CCSOMO SERVICE COORDINATORS. THEY PROVIDE GUIDANCE AND ACCESS TO RESOURCES THAT MEET THE DISTINCT NEEDS OF INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES AND THEIR FAMILIES TO HELP MAXIMIZE THEIR POTENTIAL AND STRENGTHEN THEIR LEVEL OF INDEPENDENCE. WE PROVIDED OVER 23,869 HOURS OF TCM SERVICES FOR 901 INDIVIDUALS AND THEIR FAMILIES DURING THE FISCAL YEAR ENDING 6/30/21.
MATERNAL AND FAMILY PROGRAMS PROVIDE BOTH RESIDENTIAL AND NONRESIDENTIAL SERVICES TO PREGNANT WOMEN AND FAMILIES FACING UNPLANNED PREGNANCIES. LIFEHOUSE CRISIS MATERNITY HOME PROVIDES TRANSITIONAL HOUSING AND ONSITE COMPREHENSIVE SERVICES TO HOMELESS PREGNANT WOMEN AND THEIR YOUNG CHILDREN FOR UP TO A YEAR AFTER DELIVERY. LIFEHOUSE ON AVERAGE SERVES 27 HOMELESS WOMEN, THEIR INFANTS AND YOUNG CHILDREN A YEAR. THE AFTERCARE PROGRAMS ASSISTS WOMEN AND CHILDREN AS THEY TRANSITION TO PERMANENT HOUSING PROVIDING SERVICES THAT HELP THEM TO ACHIEVE THEIR LONG-TERM GOAL OF SELF-SUFFICIENCY. THE NONRESIDENTIAL PROGRAM PROVIDES PREGNANCY AND PARENTING SUPPORT THROUGHOUT SOUTHERN MISSOURI AND SERVED OVER 80 HOUSEHOLDS IN THE 2021 FISCAL YEAR.
