ALL MEMBERS ARE REFERRED TO THE PROGRAM BY THEIR DOCTORS TO ADDRESS CARDIAC, PULMONARY, DIABETIC AND RELATED DISEASES FOR REHAB AND ON-GOING HEALTH MAINTENANCE. PARTICIPANTS REMAIN IN THE PROGRAM FOR VARYING LENGHTS OF TIME, SOME FOR 3-6 MONTHS AND SOME FOR MANY YEARS TO MAINTAIN A HEALTHY LIFESTYLE AND EXERCISE ROUTINE.
