IN ADDITION, PREGNANT HISPANIC WOMEN AND THEIR NEWBORNS, IN THE INTERCONCEPTIONAL PERIOD, RECEIVE CASE MANAGEMENT SERVICES AND ARE PROVIDED WITH INFORMATION/COUNSELING AND REFERRALS TO ADDRESS THEIR RISK FACTORS SPECIFIC TO PRENATAL CARE AND HEALTHFUL BEHAVIOURS TO IMPROVE PREGNANCY OUTCOMES.
