INNOVAGE GREATER COLORADO PACE OPERATES A PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE) IN THE DENVER, COLORADO METROPOLITAN AREA, PUEBLO, COLORADO, AND IN SAN BERNARDINO, CALIFORNIA. PACE PROVIDES PARTICIPANTS WITH COORDINATED SENIOR CARE AND SUPPORT PLANS THAT INCLUDE HEALTHCARE, IN-HOME SERVICES, TRANSPORTATION, AND SOCIAL OPPORTUNITIES TO HELP AGING ADULTS LIVE AS INDEPENDENTLY AS POSSIBLE IN THEIR OWN HOMES, FOR AS LONG AS POSSIBLE. PACE INTERDISCIPLINARY TEAMS PERFORM INDIVIDUAL ASSESSMENTS AND CUSTOMIZE A COMPREHENSIVE CARE PLAN THAT INCLUDES A NETWORK OF PRIMARY CARE AND SPECIALIST PHYSICIAN SERVICES.
