THE CLINIC SERVES IN A SUPPORTATIVE PARTNERSHIP WITH MEDICAL, EDUCATIONAL AND PSYCHO-SOCIAL SERVICES FOR CHILDREN WITH SPECIAL HEALTH CARE NEEDS, SO THAT TOTAL CARE IS PROVIDED IN A CONTEXT THAT IS FAMILY CENTERED, COMMUNITY BASED AND CULTURALLY COMPETENT. THE CLINIC PROVIDES A COMPREHENSIVE MULTI-DISCIPLINARY EVALUATION FOR MEDICALLY FRAGILE INFANTS, CHILDREN AND TEENS; DETECTS DEVELOPMENTAL DELAYS EARLY, DEVELOPS A TREATMENT PLAN THAT IS FAMILY CENTERED AND ASSISTS WITH THE UTILIZATION OF COMMUNITY RESOURCES AS NEEDED. THE CLINIC FOLLOWS EACH PROBLEM TO ENSURE THAT THE CHILD RECEIVES CORRECTIVE TREATMENT. CHILDREN ARE REFERRED TO THE CLINIC FROM MEDICAL PROFESSIONALS, PARENTS,FAMILY, FRIENDS OR COMMUNITY/STATE AGENCIES.
