THE CLINIC SERVES IN A SUPPORTATIVE PARTNERSHIP WITH MEDICAL, EDUCATIONAL AND PSYCHO-SOCIAL SERVICES FOR CHILDREN WITH SPECIAL HEALTH CARE NEEDS, SO THAT TOTAL CARE IS PROVIDED IN A CONTEXT THAT IS FAMILY CENTERED, COMMUNITY BASED AND CULTURALLY COMPETENT. THE CLINIC PROVIDES A COMPREHENSIVE MULTI-DISCIPLINARY EVALUATION FOR MEDICALLY FRAGILE INFANTS, CHILDREN AND TEENS; DETECTS DEVELOPMENTAL DELAYS EARLY, DEVELOPS A TREATMENT PLAN THAT IS FAMILY CENTERED AND ASSISTS WITH THE UTILIZATION OF COMMUNITY RESOURCES AS NEEDED.
