ADULT LIFE ENRICHMENT: SERVICES ARE OFFERED MONDAY-FRIDAY AT TWO LOCATIONS WHERE ADULTS WITH MILD TO SEVERE INTELLECTUAL AND/OR DEVEL- OPMENTAL DISABILITIES CAN TAKE PART IN A VARIETY OF ACTIVITIES FOCUSED ON COMMUNITY INTEGRATION. WE HELP ADULTS LIVE MORE INDEPEN- DENTLY THROUGH EDUCATION, LIFE SKILLS AND PRE-VOCATIONAL TRAINING, AND COMMUNITY INVOLVEMENT. WE ALSO ASSIST YOUNG ADULTS IN TRANS- ITIONING FROM THE SUPPORTS THEY RECEIVED IN PUBLIC SCHOOLS TO LIFE AFTER GRADUATION WHEN THOSE SUPPORTS ARE NO LONGER AVAILABLE.
CASE MANAGEMENT: OUR CASE MANAGEMENT SERVICES ASSIST INDIVIDUALS WITH I/DD AND THEIR FAMILIES IN NAVIGATING THE COMPLEX WORLD OF SOCIAL SERVICES, EDUCATION, FUNDING, AND FUTURE PLANNING. CASE MANAGERS PROVIDE INFORMATION ABOUT AVAILABLE RESOURCES, ASSISTANCE WITH ACCESSING THOSE RESOURCES, AND HELP FAMILIES MAKE WELL-INFORMED DECISIONS ON BEHALF OF THE PERSON IN THEIR CARE. CASE MANAGERS ALSO ADVOCATE IN THE SCHOOLS AND IN THE COMMUNITY ON BEHALF OF PEOPLE WITH I/DD AND THEIR FAMILIES. THEY HOLD FREQUENT INFORMATION SESSIONS FOR PARENTS, CARE GIVERS, PROFESSIONALS, AND EDUCATORS TO INCREASE AWARENESS OF THE ISSUES FACING PEOPLE WITH SPECIAL NEEDS.
