DAYBREAK - ADULT DAY HEALTH CENTERTHIS PROGRAM PROVIDES RESPITE FOR CAREGIVERS. THE PROGRAM PROVIDES A SUPERVISED, PROTECTIVE ENVIRONMENT, HEALTH MONITORING, DIGNIFIED ASSISTANCE AND SUPPORT WITH PERSONAL CARE NEEDS, AND NUTRITIONALLY BALANCED MEALS AND SNACKS.
CARE MANAGEMENT- A PROFESSIONAL SERVICE THAT CONDUCTS A STRINGENT ASSESSMENT OF CONSUMERS NEEDS/ABILITIES AND DEVELOPS A PLAN OF CARE TO ASSIST THEM IN MAINTAINING THEIR INDEPENDENCE WHILE CONTINUING TO LIVE AT HOME. CARE MANAGERS SERVE AS COORDINATORS AND ADVOCATES OF CARE THROUGHOUT THE CONTINUUM OF CARE.
THERAPY- THE THERAPY PROGRAM RESTORES SENIOR CLIENTS TO THEIR HIGHEST LEVEL OF FUNCTION THROUGH PHYSICAL THERAPY AND SPEECH THERAPY. THE PROGRAM PROVIDES A FULLY EQUIPPED TREATMENT FACILITY AND IS DIRECTED BY A PROFESSIONAL PHYSICAL THERAPIST.
IN HOME SERVICE - THIS PROGRAM IS PRIMARILY FUNDED THROUGH GRAND GATEWAY AREA AGENCY ON AGING WHICH ADMINISTERS GRANTS FROM THE FEDERAL GOVERNMENT UNDER THE TITLE III OF THE OLDER AMERICANS ACT OF 1965, AND THE STATE OF OKLAHOMA. THE PROGRAM PROVIDES A HOUSEKEEPER FOR PEOPLE SIXTY YEARS OF AGE OR OLDER.BOHON HEALTH CLINIC- PROVIDES THE SERVICES OF AN ON-STAFF PHYSICIAN SPECIALIZING IN THE CARE OF OLDER ADULTS.DAYBREAK- THIS PROGRAM PROVIDES RESPITE FOR CAREGIVERS.
