OUR SERVICES ARE CAREFULLY TAILORED TO PROMOTE HEALTH OF INDIVIDUALS NAVIGATING THE COURSE OF LIFE'S JOURNEY, OFFERING IN-HOME CARE AND THREE RESIDENTIAL COMMUNITIES.

NOTRE DAME HEALTH SYSTEM OPERATES SKILLED NURSING FACILITIES (CHATEAU DE NOTRE DAME, WYNHOVEN HEALTH CARE CENTER, AND OUR LADY OF WISDOM HEALTH CARE CENTER) WITH A TOTAL OF 462 DUALLY CERTIFIED MEDICARE AND MEDICAID BEDS. THE CHATEAU DE NOTRE DAME FACILITY ("CHATEAU") IS LICENSED FOR 171 BEDS (13 OF THE BEDS MAKE UP THE MEMORY CARE UNIT). THE WYNHOVEN HEALTH CARE FACILITY ("WYNHOVEN") IS LICENSED FOR 188 BEDS (22 OF THE BEDS MAKE UP THE MEMORY CARE UNIT) AND THE OUR LADY OF WISDOM HEALTH CARE FACILITY ("WISDOM") IS LICENSED FOR 138 BEDS. THE AVERAGE DAILY CENSUS FOR MEDICARE WAS 23.60 AT CHATEAU, 7.13 AT WYNHOVEN, AND 10.07 AT WISDOM. THE AVERAGE DAILY CENSUS FOR MEDICAID WAS 83.43 AT CHATEAU, 81.03 AT WYNHOVEN, AND (CONTINUED ON SCHEDULE O) 73.87 AT WISDOM. THE AVERAGE DAILY CENSUS FOR SEMI PRIVATE AND PRIVATE WAS 29.69 AT CHATEAU, 7.34 AT WYNHOVEN, AND 26.13 AT WISDOM. PHYSICAL, OCCUPATIONAL, AND SPEECH REHABILITATION THERAPY AS PRESCRIBED FOR THE INDIVIDUAL PATIENT BY THE ATTENDING PHYSICIAN ARE AVAILABLE IF NEEDED AT THE SKILLED NURSING FACILITIES. CUSTODIAL CARE AND HOSPICE SERVICES ARE ALSO PROVIDED AT CHATEAU, WYNHOVEN, AND WISDOM. THESE FACILITIES OPERATE WITH THE BELIEF THAT REMAINING MENTALLY AND PHYSICALLY ACTIVE IS THE KEY TO MAXIMIZING QUALITY OF LIFE. AS SUCH, WE ENCOURAGE RESIDENTS TO TAKE PART IN THE MANY INDIVIDUAL AND GROUP ACTIVITIES OFFERED, RANGING FROM ARTS AND CRAFTS TO READING, GAMES, PARTIES, MOVIES, THE EVER-POPULAR BINGO, AND MORE. MOST IMPORTANTLY, HOWEVER, WE BELIEVE THAT THE MOST FULFILLED LIFE IS THE ONE STRENGTHENED BY FAITH, LIVED IN A SUPPORTIVE ENVIRONMENT BUILT ON STRONG SPIRITUAL FOUNDATIONS. AT THE FOREFRONT OF OUR MISSION, NOTRE DAME HEALTH SYSTEM PROVIDES DEEP FAITH-BASED CARE TO ASSIST AND ENCOURAGE RESIDENTS OF ALL FAITHS ALONG THEIR OWN INDIVIDUAL JOURNEYS.
NOTRE DAME HOSPICE IS A MINISTRY OF THE ARCHDIOCESE OF NEW ORLEANS AND IS LICENSED TO PROVIDE HOSPICE SERVICES IN THREE DEFINED SERVICE AREAS: THE GREATER NEW ORLEANS AREA, THE MISSISSIPPI GULF COAST, AND GREATER BATON ROUGE. NOTRE DAME HOSPICE STRIVES TO PROVIDE QUALITY, COMPASSIONATE HEALTH CARE AND SUPPORT SERVICES IN A CHRISTIAN ENVIRONMENT, RESPECTING THE UNIQUENESS OF EACH PERSON, PROMOTING AND ENHANCING THE QUALITY OF LIFE, AND PRESERVING DIGNITY IN LIFE, ILLNESS AND DEATH. THE AVERAGE DAILY CENSUS FOR EACH OF THE SERVICE AREAS WAS 145.76 FOR NOTRE DAME HOSPICE (GREATER NEW ORLEANS AREA), 49.23 FOR NOTRE DAME HOSPICE OF MISSISSIPPI, AND 30.53 FOR NOTRE DAME HOSPICE OF GREATER BATON ROUGE. WE BELIEVE IN THE SACREDNESS OF LIFE FROM CONCEPTION TO NATURAL DEATH, AND WE DEVOTE (CONTINUED ON SCHEDULE O) OURSELVES TO SUPPORTING PATIENTS AND FAMILIES THROUGH THE DYING PROCESS AT THE END OF LIFE'S JOURNEY ON THIS EARTH AND NEW LIFE IN ETERNITY.
NOTRE DAME HEALTH SYSTEM OPERATES ASSISTED LIVING APARTMENTS. THE COMPLEX CONSISTS OF 100 APARTMENTS WHICH ARE RENTED UNDER PRIVATE PAY LEASE AGREEMENTS. THE AVERAGE DAILY CENSUS WAS 70.61 RESIDENTS FOR THE YEAR. THE APARTMENTS ARE SEPARATE FROM THE NURSING HOME BUT SHARE THE SAME CAMPUS. RESIDENTS ARE ABLE TO SELECT ASSISTED LIVING SERVICES AT VARYING LEVELS WHICH INCLUDE DINING, HOUSEKEEPING, BASIC MEDICATION PASS, AND PERSONAL CARE SERVICES. RESIDENTS ARE ALSO ABLE TO UTILIZE THE PHYSICAL, OCCUPATIONAL, AND SPEECH REHABILITATIVE THERAPIES THAT ARE OFFERED AT THE SKILLED NURSING FACILITY IF SUCH SERVICES ARE PRESCRIBED FOR RESIDENTS BY THEIR PERSONAL PHYSICIANS. AS WITH THE NURSING HOME PATIENTS, THE APARTMENT RESIDENTS ARE ENCOURAGED TO PARTICIPATE IN THE MANY INDIVIDUAL AND GROUP (CONTINUED ON SCHEDULE O) ACTIVITIES OFFERED AT THE APARTMENTS WHICH INCLUDE BINGO, GAMES, READING, PARTIES, MOVIES, VARIOUS OUTINGS AND ARTS AND CRAFTS. RESIDENTS ARE ALSO STRONGLY ENCOURAGED TO EXERCISE AND STRENGTHEN THEIR FAITH LIVES BY PARTICIPATING IN THE FACILITY'S SUPPORTIVE SPIRITUAL ENVIRONMENT WHICH IS DEEPLY ROOTED IN CATHOLIC FAITH AND TRADITION.
NOTRE DAME HEALTH SYSTEM INTRODUCED NOTRE DAME HOME CARE AND NOTRE DAME HOME HEALTH IN JULY 2018 AND AUGUST 2019, RESPECTIVELY. THESE PROVIDERS ARE LICENSED TO PROVIDE HOME CARE AND HOME HEALTH SERVICES TO THOSE IN NEED. NOTRE DAME HOME HEALTH OFFERS CLINICAL MEDICAL CARE PROVIDED BY SKILLED MEDICAL PROFESSIONALS SUCH AS OCCUPATIONAL THERAPISTS, PHYSICAL THERAPISTS, AND REGISTERED NURSES. WE ASSIST FAMILIES AND SENIORS WITH MEDICATIONS, INJECTIONS, MEDICAL TESTS, AND OVERALL MONITORING OF HEALTH. NOTRE DAME HOME CARE PROVIDES THE SAME LEVEL OF INTIMATE ATTENTION AND SUPPORT YOU EXPECT FROM NOTRE DAME HEALTH SYSTEM WITHOUT HAVING TO MOVE FROM THE HOME YOU CHERISH.
