NEWBORN SCREENING FOLLOW-UP PROGRAM: SCDAA-MC IS THE SUB-RECIPIENT OF A GRANT WITH THE SICKLE CELL DISEASE ASSOCIATION OF AMERICA ON THE HRSA NEWBORN SCREENING FOLLOW-UP PROGRAM GRANT TO IDENTIFY INDIVIDUALS LIVING WITH SICKLE CELL DISEASE WHO ARE IN MEDICAL CARE IN A MEDICAL HOME. THEIR CURRENT CARE QUALITY IS ASSESED AN THEY ARE LINKED TO A MEDICAL HOME. THE PROGRAM PROVIDES CARE COORDINATION FOR SICKLE CELL CLIENTS LIVING IN RURAL AND UNDERSERVED AREAS. NEWBORN SCREENING COUNSELING PROGRAM: SCDAA-MC HAS A PARTNERSHIP WITH THE ALABAMA DEPARTMENT OF PUBLIC HEALTH (ADPH) THAT ALLOWS ADPH TO PROVIDE THE ASSOCIATION WITH THE LIST OF BABIES BORN IN THE ASSOCIATIONS NINE COUNTY SERVICE AREA THAT HAVE SICKLE CELL TRAIT AND SICKLE CELL DISEASE. THE PARENTS OF THE BABIES ARE PROVIDED WITH EDUCATION. THOSE WITH SICKLE CELL DISEASE ARE REFERRED TO THE PEDIATRIC SICKLE CELL CLINIC AT THE UNIVERSITY OF SOUTH ALABAMA.
CASE MANAGEMENT PROGRAM IS DESIGNED TO ENSURE THAT ALL INVIDIVUALS WITH SICKLE CELL DISEASE HAVE ACCESS TO THE FULL ARRAY OF MEDICAL, SOCIAL, EDUCATIONAL, AND COMMUNITY SERVICES THAT ARE AVAILABLE IN OUR SERVICE AREA. CASE MANAGEMENT SERVICES ARE AVAILABLE TO HELP CLIENTS AND THEIR FAMILIES COPE FINANCIALLY AND EMOTIONALLY WITH SICKLE CELL DISEASE. OUR CASE MANAGER ASSISTS CLIENTS WITH ACCESSING MANY RESOURCES RELATING TO HOUSING, TRANSPORTATION, CHILDCARE, MEDICATION AND INSURANCE PROGRAMS, EDUCATIONAL NEEDS, CAREER ASSISTANCE, AND PSYCHO-SOCIAL ISSUES. THE ASSOCIATION ALSO PROVIDES FINANCIAL ASSISTANCE TO HELP CLIENTS PURCHASE THE MEDICATIONS AND EQUIPMENT NECESSARY FOR TREATMENT OF THE DISEASE.
AFTER-SCHOOL TUTORIAL AND SUMMER ENRICHMENT PROGRAM: SCDAA-MC PROVIDES A TUTORING PROGRAM FOR CHILDREN IN MOBILE COUNTY WITH SICKLE CELL DISEASE. THE PROGRAM IS OFFERED THREE DAYS A WEEK FOR 2.5 HOURS PER DAY. CERTIFIED TEACHERS ARE CONTRACTED TO PROVIDE TUTORING. THE SUMMER ENRICHMENT PROGRAM IS A TWO WEEK PROGRAM THAT IS AN EXTENSION OF THE AFTER-SCHOOL TUTORIAL PROGRAM. CHILDREN ARE GIVEN TWO WEEKS OF ENRICHMENT ACTIVITIES DURING THE SUMMER. THEY PARTICIPATE IN EDUCATIONAL FIELD TRIPS AND TAKE PART IN ACTIVITIES THAT SUPPLEMENT THEIR ACADEMIC EXPERIENCE.
