THE GENESIS HEALTH FOUNDATION, INC. (DOING BUSINESS AS BROOKS HEALTH FOUNDATION) IS A 501(C)(3) PUBLICLY SUPPORTED CHARITY ESTABLISHED TO SUPPORT THE MISSION OF GENESIS HEALTH, INC. (DOING BUSINESS AS BROOKS HEALTH SYSTEM) OF JACKSONVILLE, FLORIDA. PLEASE SEE SCHEDULE O FOR A CONTINUATION OF OUR PROGRAM SERVICE ACCOMPLISHMENTS.ESTABLISHED TO SUPPORT THE MISSION OF BROOKS REHABILITATION, THE BROOKS HEALTH FOUNDATION SUPPORTS THE COMMUNITY ACTIVITIES WHICH MEETS THE NEEDS OF PATIENTS AND THEIR FAMILIES THROUGH RESEARCH AND EDUCATION, CHARITY CARE, AND COMMUNITY PROGRAMS THAT ENHANCE THE QUALITY OF LIFE FOR THOSE LIVING WITH A DIFFERING ABILITY.THE FOUNDATION SEEKS TO SUPPORT BROOKS HEALTH SYSTEM IN THE FOLLOWINGWAYS:1. SUPPORT OF THE OPERATION OF BROOKS HEALTH SYSTEM AND ITS INPATIENT AND OUTPATIENT FACILITIES AND PROGRAMS IN THE CARE AND TREATMENT OF THE SICK, DISABLED AND INJURED.2. SUPPORT OF THE OPERATION OF BROOKS HEALTH SYSTEM IN THE CHARITY CARE PROVIDED TO THE SICK, DISABLED AND INJURED.3. SUPPORT OF THE EDUCATION AND TRAINING IN THE CARE AND TREATMENT OF THE SICK, DISABLED AND INJURED.4. SUPPORT OF EDUCATIONAL, SCIENTIFIC AND MEDICAL RESEARCH, RELATED TO THE CARE AND TREATMENT OF SICK, DISABLED AND INJURED.5. PROMOTION AND SUPPORT OF COMMUNITY PROGRAMS RELATED TO THE PREVENTION OF INJURIES AND ILLNESSES ADDRESSED THROUGH REHABILITATION AND RELATED MEDICAL SERVICES.THE BROOKS HEALTH FOUNDATION SUPPORTS THE OPERATION OF BROOKS HEALTH SYSTEM'S PROGRAMS AND SERVICES THROUGH FUNDING RECEIVED FROM INDIVIDUAL DONATIONS, GRANTS AND OTHER FUNDRAISING EVENTS AND INITIATIVES.IN 2021, THE BROOKS HEALTH FOUNDATION RECEIVED APPROXIMATELY $540,000 IN DONATIONS TO SUPPORT BROOKS COMMUNITY PROGRAMS. BROOKS FOUNDATION IS GRATEFUL FOR THE SUPPORT OF ITS CONTRIBUTORS.THE ANNUAL BROOKS GOLF CLASSIC FUNDRAISING EVENT RAISED APPROXIMATELY $227,000, NET, IN SUPPORT OF BROOKS HEALTH SYSTEM'S PROGRAMS AND SERVICES. THE ANNUAL BROOKS PAR-TEE FUNDRAISING EVENT DID NOT OCCUR IN 2021 DUE TO THE COVID19 PANDEMIC. THE W.H.E.E.L FUND, "WE'RE HELPING EVERYONE EXPERIENCE LIFE," IS AN EMPLOYEE GIVING PROGRAM SUPPORTED THROUGH STAFF MEMBERS' DONATIONS. THE FUND PROVIDES ASSISTANCE TO BROOKS CHARITY PATIENTS. IN 2021, BROOKS EMPLOYEES DONATED APPROXIMATELY $106,000 TO PROVIDE UNINSURED AND UNDERUNSURED PATIENTS FREE MEDICALLY NECESSARY EQUIPMENT AND MEDICATIONS.PROGRAMS AND SERVICES IMPACTED BY THE GENEROSITY OF DONORS AND EMPLOYEES:RESEARCH: THE CLINICAL RESEARCH CENTER (CRC) AT BROOKS REHABILITATION IS COMMITTED TO THE INVESTIGATION OF INNOVATIVE TREATMENT PROTOCOLS AND THE UTILIZATION OF NEW TECHNOLOGIES THAT WILL HELP OUR PATIENTS MAXIMIZE FUNCTION AND ACHIEVE THEIR HIGHEST LEVEL OF INDEPENDENCE.NEURO RECOVERY CENTER:THE BROOKS REHABILITATION NEURO RECOVERY CENTER (NRC) OFFERS SPECIALIZED EQUIPMENT FOR CUSTOMIZED REHABILITATION. THIS UNIQUE GYM ALLOWS INDIVIDUALS WITH DISABILITIES TO CONTINUE ONGOING EXERCISE AND CONDITIONING TO MAINTAIN AND IMPROVE FUNCTIONAL MOVEMENT AND ABILITIES.SCHOOL RE-ENTRY:THE BROOKS SCHOOL RE-ENTRY PROGRAM (BSRP) IS AN ONGOING PROGRAM WHICH MAXIMIZES A CHILD'S SUCCESSFUL TRANSITION BACK TO SCHOOL FOLLOWING A DISABLING ILLNESS OR INJURYADAPTIVE SPORTS & RECREATION:BROOKS REHABILITATION OFFERS THE MOST COMPREHENSIVE ADAPTIVE SPORTS AND RECREATION PROGRAM IN THE COUNTRY, PROVIDING FUN AND FITNESS FOR PEOPLE OF ALL AGES LIVING WITH A PHYSICAL DISABILITY. PROGRAM PARTICIPANTS CAN ENJOY COMPETITIVE OR RECREATIONAL TEAM AND INDIVIDUAL SPORTS, PLUS A VARIETY OF OTHER WELLNESS AND FITNESS ACTIVITIES.BRAIN INJURY CLUBHOUSE:THE BROOKS CLUBHOUSE IS A COMMUNITY HEALTH PROGRAM THAT PROVIDES FOR THE LONG-TERM RECOVERY NEEDS OF INDIVIDUALS WHO HAVE SUFFERED AN ACQUIRED BRAIN INJURY. THIS DAY-PROGRAM BRIDGES THE GAP BETWEEN MEDICAL REHABILITATION, VOCATIONAL TRAINING, AND COMMUNITY REINTEGRATION. IT IS CURRENTLY THE ONLY BRAIN INJURY CLUBHOUSE IN FLORIDA AND ONE OF ONLY 24 IN THE WORLD.BROOKS REHABILITATION APHASIA CENTER:THE APHASIA CENTER PROVIDES COMPREHENSIVE SUPPORT FOR PARTICIPANTS AFFECTED BY APHASIA DUE TO STROKE, TRAUMATIC BRAIN INJURY, BRAIN TUMORS, OR OTHER NEUROLOGICAL DISORDERS. THE APHASIA CENTER OFFERS TWO PROGRAMS, INCLUDING A COMMUNITY TRACK AND AN INTENSIVE TRACK.PEDIATRIC RECREATION:THE PEDIATRIC RECREATION PROGRAM OFFERS A SUPPORTIVE ENVIRONMENT FOR YOUTH WITH PHYSICAL AND/OR DEVELOPMENTAL DISABILITIES. THE PROGRAM INCREASES SOCIALIZATION THROUGH ACTIVITIES THAT FOSTER A HEALTHY LIFESTYLE AND COMMUNITY CONNECTION. FAMILY SUPPORT IS ALSO AVAILABLE FOR THOSE SEEKING OPPORTUNITIES FOR THEIR CHILDREN OUTSIDE OF TRADITIONAL THERAPIES.WELLNESSTHE BROOKS REHABILITATION WELLNESS PROGRAM FOCUSES ON FITNESS AND WELLNESS EDUCATION FOR INDIVIDUALS WHO EXPERIENCED NEUROLOGICAL DISEASE OR INJURY IN PARTNERSHIP WITH THE FIRST COAST YMCA. PARTICIPANTS RECEIVE AN ASSESSMENT FROM PHYSICAL THERAPISTS AND A CUSTOMIZED FITNESS PLAN TO MAINTAIN MOBILITY.
