SELF RELIANCE, INC. PROVIDES FIVE CORE SERVICES TO INDIVIDUALS WITH DISABILITIES. THE FOLLOWING SERVICES WERE PERFORMED IN FY 2022: 1. ADVOCACY AND LEGAL SERVICES: 211 CONSUMERS SERVED. 2. PEER SUPPORT AND MENTORING: 63 CONSUMERS SERVED. 3. INDEPENDENT LIVING SKILLS AND TRAINING: 1061 CONSUMERS SERVED. 4. INFORMATION AND REFERRAL: 2012 CONSUMERS SERVED. 5. TRANSITION SERVICES: 84 CONSUMERS SERVED. IN ADDITION,3 CONSUMERS RECEIVED ASSISTIVE TECHNOLOGY AND 9 CONSUMERS RECEIVED HOME MODIFICATION SERVICES.
