THE MISSION OF THE COMMUNITY ADVANCED PRACTICE NURSES (CAPN) IS TO STRENGTHEN THE LIVES OF PERSONS WHO ARE MEDICALLY UNDERSERVED AND TO HELP INTERRUPT THE CYCLE OF POVERTY AND HOMELESSNESS FOR THESE INDIVIDUALS.

COMMUNITY ADVANCED PRACTICE NURSES, INC. IS A NON-PROFIT FREE CLINIC FOR INDIVIDUALS WHO HAVE LITTLE TO NO ACCESS TO HEALTH CARE IN METROPOLITAN ATLANTA. CAPN PROVIDES FREE-OF-CHARGE HEALTH CARE SERVICES TO ATLANTA'S HOMELESS, UNINSURED, AND LOW INCOME POPULATION. THE CLINIC PLACES SPECIAL EMPHASIS ON SERVING WOMEN, CHILDREN AND YOUTH. CAPN OFFERS A BROAD RANGE OF HEALTH CARE SERVICES INCLUDING IMMUNIZATIONS, WELL- AND SICK- CARE, FAMILY PLANNING, PATIENT EDUCATION, FREE PRESCRIPTION AND OVER THE COUNTER MEDICATIONS, CASE MANAGEMENT, ACUTE AMBULATORY CARE, REFERRALS, AND MENTAL HEALTH CARE. CAPN PROVIDES SERVICES AT FOUR (4) STRATEGICALLY LOCATED, SHELTER-BASED SITES WITHIN METROPOLITAN ATLANTA'S MOST UNDERSERVED AND HIGH RISK AREAS.IN 2021, THE ORGANIZATION SERVED 2,948 UNDUPLICATED INDIVIDUALS. 52% OF PATIENTS WERE CHILDREN AND YOUTH (24 AND UNDER), AND 65% OF ADULT PATIENTS WERE FEMALE.
