ICF-IID GROUP HOMES - MACON ICF-IID GROUP HOME, IOTLA STREET GROUP HOME, SMOKEY ICF-IID, AND WEBSTER GROUP HOME EACH SERVE SIX PERSONS WITH DEVELOPMENTAL DISABILITIES WHO NEED ICF-IID LEVEL OF CARE. UTILIZATION REVIEW IS DONE AT LEAST EVERY 180 DAYS TO ENSURE APPROPRIATE USE OF BEDS. THE 24 ICF-IID RESIDENTS BENEFIT FROM ACTIVE TREATMENT ON A CONTINUING BASIS. THE ICF'S EMPLOY TEN TO TWELVE FTE'S. COMPLETE ROOM AND BOARD ARE PROVIDED 24 HOURS PER DAY, 365 DAYS A YEAR WITH 24 HOUR AWAKE STAFF. STAFF ASSIST WITH ACTIVITIES OF DAILY LIVING AND PROVIDE TRAINING ON A CONTINUAL BASIS. TRANSPORTATION IS FURNISHED.
MACON CITIZENS ENTERPRISES - MCE IS A COMMUNITY REHABILITATION PROGRAM WHICH SERVES APPROXIMATELY 53 CONSUMERS. MCE OFFERS EVALUATION, ADVP, AND COMMUNITY BASED SERVICES. PROGRAMS ARE INDIVIDUALIZED TO SERVE CONSUMERS IN THE MOST APPROPRIATE MANNER POSSIBLE AND ADAPTATIONS AND ACCOMMODATIONS ARE MADE AS NECESSARY. MCE PROVIDES COMMUNITY-BASED DAY SERVICES. TRANSPORTATION IS FURNISHED. MCH PROVIDES ALTERNATIVE FAMILY LIVING (AFL) SERVICES TO 9 INDIVIDUALS IN 1 LICENSED 3 BED HOME AND 5 UNLICENSED ONE BEDROOM HOMES IN THE COMMUNITY.
DDA GROUP HOME - HARRISON AVENUE GROUP HOME SERVES SIX ADULT CONSUMERS 24 HOURS, 365 DAYS PER YEAR. ALTHOUGH THE FACILITY IS STAFFED 24 HOURS PER DAY, THIRD SHIFT IS A SLEEP OVER SHIFT. RESIDENTS ARE TAUGHT ACTIVITIES OF DAILY LIVING SKILLS WHICH HELP PROMOTE INDEPENDENCE. ROOM AND BOARD ARE FURNISHED. TRANSPORTATION IS FURNISHED.
INDEPENDENT LIVING - THE YONCE HOUSE IS AN INDEPENDENT LIVING PROGRAM WHICH HOUSES THREE PERSONS. BECAUSE OF PHYSICAL AND HEALH IMPAIRMENTS, THE HOUSE IS STAFFED TWENTY-FOUR HOURS PER DAY ALTHOUGH THIRD SHIFT IS A SLEEP-OVER POSITION. RESIDENTS CONTINUE TO TRAIN IN AREAS TO PROMOTE INDEPENDENCE. ROOM AND BOARD ARE FURNISHED AS IS TRANSPORTATION.
