HOSPICE OPERATIONS THROUGH SEPTEMBER 10, 2021 AND PRIOR TO THE SALE PROVIDED 191,155 HOSPICE ROUTINE VISITS. JOURNEY HEALTH FOUNDATION FKA HOSPICE OF CHATTANOOGA WAS THE ONLY AREA HOSPICE THAT EMPLOYED EIGHT HIGHLY TRAINED PHYSICIANS, 175 REGISTERED NURSES, NURSE PRACTICIONERS, AND CERTIFIED NURSING ASSISTANTS WHOSE JOB IT WAS TO TAKE CARE OF PATIENTS. HOSPICE OFFERED BEREAVEMENT SERVICES TO FAMILY MEMBERS OF THEIR PATIENTS AND OTHERS. HOSPICE PROVIDED CARE THROUGH A TEAM APPROACH. CARE WAS PROVIDED IN 18 COUNTIES IN THE TENNESSEE AND NORTH GEORGIA AREA, INCLUDING CARE IN THE PATIENT'S HOME, IN ACUTE AND EXTENDED CARE FACILITIES, AND ASSISTED LIVING HOMES. CONSISTENT WITH THE ORGANIZATION'S MISSION, CARE WAS PROVIDED TO PATIENTS REGARDLESS OF THEIR ABILITY TO PAY. PATIENTS WHO MET THE ORGANIZATION'S CRITERIA FOR CHARITY CARE WERE PROVIDED CARE WITHOUT CHARGE OR AT AMOUNTS LESS THAN ESTABLISHED RATES.
