TANDEM HEALTH SC (THS) IS A 501(C)(3) NOT-FOR-PROFIT COMMUNITY HEALTH CENTER LOCATED IN SUMTER, SOUTH CAROLINA. OUR ORGANIZATIONAL MISSION AND VISION IS FOCUSED ON THE CONTINUOUS PURSUIT OF ADDRESSING, NOT ONLY THE SHORTAGE OF HEALTHCARE SERVICES WITHIN SUMTER COUNTY, BUT ALSO THE SOCIAL DETERMINANTS THAT OFTEN PREVENT PATIENTS FROM ACCESSING AND/OR FULLY UTILIZING THE HEALTH RESOURCES MADE AVAILABLE TO THEM. TODAY, THS EMPLOYS, IN ADDITION TO ITS 40 PLUS MEDICAL PROVIDERS, A VARIETY OF HEALTHCARE PROFESSIONALS TO ASSIST PATIENTS WITH SUCH THINGS AS MEDICAL CASE MANAGEMENT, NUTRITION, BEHAVIORAL HEALTH, TRANSPORTATION, AND SUBSTANCE ABUSE DISORDERS.
