THE CARESOURCE FOUNDATION FURTHERS THE CHARITABLE PURPOSES OF CARESOURCE OHIO, INC. AND ITS RELATED ENTITIES ("CARESOURCE FAMILY OF COMPANIES") BY PROVIDING ADVANCED, INNOVATIVE SOLUTIONS AND ADDRESSING HEALTH CARE NEEDS OF THE UNDERSERVED THROUGH GRANTS, OUTREACH, MEDICAL EXPERTISE, STRATEGIC COMMUNITY PARTNERSHIPS, AND VOLUNTEERISM. THE CARESOURCE FOUNDATION FUNDS COMMUNITY GRANTS TO NONPROFIT ORGANIZATIONS THAT IMPACT CHILDREN, ADULTS, AND FAMILIES IN AREAS THAT IMPROVE HEALTH EQUITY, HEALTH OUTCOMES, AND HEALTH LITERACY. SPECIFICALLY, THE CARESOURCE FOUNDATION TARGETS THE FOLLOWING: SUPPORTING HEALTH EDUCATION AND PROMOTING HEALTHY HABITS; IMPROVING MATERNAL, INFANT, AND CHILD HEALTH; ENSURING FAMILIES HAVE SAFE, STABLE, AND HEALTHY HOMES AND NEIGHBORHOODS; AND STRENGTHENING THE CAPACITY OF A DIVERSE HEALTH AND 21ST CENTURY WORKFORCE. IN 2021, THE CARESOURCE FOUNDATION PROVIDED GRANTS TO 106 NONPROFIT ORGANIZATIONS ACROSS EIGHT STATES TOTALING $3.9 MILLION. SINCE 2006, THE CARESOURCE FOUNDATION HAS AWARDED MORE THAN $25.1 MILLION IN CHARITABLE ASSISTANCE TO STRENGTHEN THE NETWORK OF HEALTH AND HUMAN SERVICE NONPROFIT ORGANIZATIONS THROUGHOUT THE STATES IN WHICH THE CARESOURCE FAMILY OF COMPANIES DO BUSINESS. THE CARESOURCE FOUNDATION PROVIDES FUNDING TO NONPROFITS WHO ARE WORKING TO ELIMINATE POVERTY, PROVIDING MUCH-NEEDED SERVICES TO LOW-AND MODERATE-INCOME FAMILIES, BUILDING HEALTHIER COMMUNITIES, AND DEVELOPING INNOVATIVE APPROACHES TO ADDRESS CRITICAL ISSUES OF CHILDREN, ADULTS, AND FAMILIES. THE CARESOURCE FOUNDATION IS A LEADER IN CULTIVATING PARTNERSHIPS THAT ENHANCE AND IMPROVE THE HEALTH CARE AND LIFE SERVICES FOR CONSUMERS. ALL OF THESE GRANTS HAVE BEEN SELECTED THROUGH A RIGOROUS REVIEW PROCESS THAT EVALUATES INNOVATION, IMPACT, AND SUSTAINABILITY. OVER THE LAST DECADE, THE ORGANIZATION HAS SEEN GREAT CHALLENGES IN THE AREAS OF WELL-BEING AND HEALTH CARE. WHAT IS MOST EXCITING, HOWEVER, IS THE IMPACT THE CARESOURCE FOUNDATION HAS HAD AS THE RESULT OF PARTNERSHIPS WITH SOME OF THE MOST IMPRESSIVE NONPROFITS IN THE COUNTRY. THE CARESOURCE FOUNDATION SIGNATURE GRANTS ARE FOCUSED ON INNOVATIVE, HIGH-IMPACT APPROACHES TO SOLVING CRITICAL SOCIAL ISSUES. THE CARESOURCE FOUNDATION AWARDED A TOTAL OF $2.1 MILLION IN SIGNATURE GRANTS TO THE FOLLOWING ORGANIZATIONS IN 2021: - NURSE-FAMILY PARTNERSHIP NATIONAL OFFICE (DENVER, CO): $529,034 TO BE GIVEN OVER 3 YEARS FOR A CULTURAL CONSCIOUSNESS PATHWAY TO DEVELOP, PILOT, IMPLEMENT, AND EVALUATE A CULTURAL CONSCIOUSNESS PATHWAY FOR NURSES IN OUR NATIONAL NETWORK OF LOCAL PARTNERS. - PHOEBE FOUNDATION INC. (ALBANY, GA): $256,226 TO BE GIVEN OVER 3 YEARS FOR THE NURSE-FAMILY PARTNERSHIP EXPANSION TO ESTABLISH A NURSE FAMILY PARTNERSHIP AFFILIATE IN DOUGHERTY COUNTY IN PARTNERSHIP WITH THE PHOEBE-PUTNEY HEALTH SYSTEM. - ONEFIFTEEN RECOVERY (DAYTON, OH): $220,000 TO BE GIVEN OVER 2 YEARS FOR TECH-ENABLED CARE FOR RURAL OHIO TO BUILD AND ESTABLISH A RURAL TELEHUB PROGRAM THAT WILL CONNECT RURAL PATIENTS TO SUBSTANCE USE DISORDER ("SUD") CLINICIANS VIRTUALLY AND DELIVER VIRTUAL ADDICTION MEDICINAL CARE. - OHIO STATE UNIVERSITY FOUNDATION (DAYTON, OH): $125,000 TO BE GIVEN OVER 2 YEARS FOR MOMS2B AND DADS2B, A WEEKLY, NEIGHBORHOOD-BASED GROUP PROGRAM THAT HELPS TO CONNECT WOMEN TO MEDICAL CARE AND TO LEARN SKILLS THAT IMPROVE PREGNANCY AND BIRTH OUTCOMES. - OPEN HAND ATLANTA INC. (ATLANTA, GA): $375,000 FOR MEDICALLY-TAILORED MEALS INTERVENTION FOR PREGNANT WOMEN WITH GESTATIONAL DIABETES TO SEEK TO DETERMINE IF THE INTERVENTION AND SUPPORT GROUPS CAN REDUCE BIRTH COMPLICATIONS AND INCREASE MATERNAL AND INFANT HEALTH OUTCOMES. - EVSC FOUNDATION INC.
