AIR FORCE VILLAGES, INC, (DBA BLUE SKIES OF TEXAS (BST)) AND AFFILIATES IS A TEXAS NOT-FOR-PROFIT, CONTINUING-CARE RETIREMENT COMMUNITY (CCRC) PROVIDING SERVICES AND CARE FOR SENIORS THROUGHOUT THE FULL CONTINUUM INCLUDING INDEPENDENT LIVING, ASSISTED LIVING, DEMENTIA CARE, NURSING, AND HOSPICE. THE ORGANIZATION IS GOVERNED BY THE BST BOARD OF DIRECTORS, WHICH ELECTS THE SEPARATE BOARDS OF THE OTHER ENTITIES. BST OPERATES MANY OF THE ADMINISTRATIVE FUNCTIONS OF THE ORGANIZATION, INCLUDING STRATEGIC PLANNING, MARKETING, DEVELOPMENT, FUNDRAISING, FINANCE, TECHNOLOGY, AND RISK MANAGEMENT.SEE CONTINUATION ON SCHEDULE O SUPPORTING ORGANIZATIONS ARE BST SENIOR LIVING EAST, INC. BST WEST HAS DEVELOPED APPROXIMATELY 200 ACRES OF ITS 660-ACRE TRACT. ALL FACILITIES AND SERVICES ARE FULLY OPERATIONAL.BST EAST CONSISTS OF TWO RESIDENTIAL TOWERS WITH 228 APARTMENTS; THREE FREE-STANDING APARTMENT BUILDINGS CALLED THE HILL RESIDENCES CONSISTING OF 36 TOTAL APARTMENTS; 24 GARDEN HOMES; A VILLAGE TOWN CENTER WITH FRONT DESK SERVICES, LOBBY, MAIL FACILITY, DINING ROOM, MEETING ROOMS, LIBRARY AND MUSIC ROOM. IN ADDITION, COCHRANS CAFE AND BISTRO IS LOCATED ON THE MAIN LEVEL AND THE SKY LOUNGE, A FULL SERVICE RESTURANT AND BAR, IS LOCATED ON THE TOP 16TH FLOOR. BSTE LEASES TO HSC A 23-UNIT ASSISTED-LIVING FACILITY KNOWN AS ARNOLD HOUSE, AND LEASES TO AFV HEALTH SERVICES, INC, A 47-BED HEALTHCARE CENTER, KNOWN AS THE MISSON, IN BEXAR COUNTY, TEXAS.BST EAST ALSO INCLUDES TWO CHAPELS, ARTS AND CRAFTS CENTER, FITNESS CENTER, SWIMMING POOL, BEAUTY SALON, WOODWORKING HOBBY SHOP, AND GARDEN PLOTS. ALL FACILITIES AND SERVICES ARE FULLY OPERATIONAL.EACH NURSING HOME RESIDENT MUST RETAIN THE SERVICES OF A PERSONAL PHYSICIAN WHO IS RESPONSIBLE FOR SUPERVISING HIS/HER MEDICAL AND OVERALL HEALTH CARE. RESIDENTS MUST REMAIN UNDER THE CARE OF A PHYSICIAN AT ALL TIMES WHILE IN THE FACILITY. THE PHYSICIAN IS REQUIRED TO MEET STATE AND FEDERAL REQUIREMENTS WITH RESPECT TO SERVICES, FREQUENCY OF VISITS, AND AVAILABILITY FOR EMERGENCY CARE. BST WEST ALSO RETAINS DIETARY AND PHARMACY CONSULTANTS SINCE NEARLY ALL RESIDENTS ARE ELIGIBLE FOR MILITARY MEDICAL BENEFITS, SERIOUS MEDICAL CONDITIONS ARE SEEN AT NEARBY MILITARY AND CIVILIAN MEDICAL FACILITIES.BLUE SKIES OF TEXAS AND ITS AFFILIATES SHARE A COMMON PAY MASTER FOR ITS EMPLOYEES.
