PROVIDE MEDICAL AND NON-MEDICAL CASE MANAGEMENT SERVICES INCLUDING ELIGIBILITY/REFERRAL FOR MENTAL HEALTH SERVICES; SUBSTANCE ABUSE SERVICES; TREATMENT ADHERENCE COUNSELING; HOME HEALTH CARE SERVICES; EMERGENCY FINANCIAL SERVICES ASSISTANCE; FOOD SERVICES; HOUSING ASSISTANCE; MEDICAL/INSURANCE PREMIUM PAYMENTS; AND MEDICAL TRANSPORTATION TO APPROXIMATELY 10,460 CLIENTS.
