INDIVIDUALS WITH A BRAIN INJURY ARE SERVED THROUGH A COMMUNITY-BASED CASE MANAGEMENT SYSTEM THAT SUPPORTS PERSON-CENTERED PLANNING AND SURVIVOR DIRECTED APPROACH.
