ADMINISTRATION FOR COMMUNITY LIVING - PROVIDES INFORMATION AND REFERRAL, PEER SUPPORT, ADVOCACY, TRANSITION SUPPORT, AND INDEPENDENT LIVING SKILLS TRAINING TO PEOPLE WITH DISABILITIES.
SUPPORT FOR INDEPENDENT LIVING - PROVIDES LINKAGES TO COMMUNITY-BASED PROGRAMS FOR PEOPLE WITH DISABILITIES AS WELL AS FINANCIAL ASSISTANCE FOR THE PURCHASE OF ADAPTIVE EQUIPMENT AND HOME MODIFICATIONS THAT REMOVE ARCHITECTURAL BARRIERS IN THEIR HOMES.
SUPPORT COORDINATOR - ASSISTS DDD ELIGIBLE INDIVIDUALS IN ACCESSING COMMUNITY-BASED SUPPORTS AND SERVICES TO ENHANCE THEIR INDEPENDENT LIVING BY IDENTIFYING APPROPRIATE PROGRAMS AND PROVIDERS, FACILITATING SOCIALIZATION SKILLS, AND BUILDING LIFE EXPERIENCES WITH THE ULTIMATE GOAL OF SECURING EMPLOYMENT.
