HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) PROJECTS-INNOVATIVE SOLUTIONS IS THE DEVELOPMENT OF A RURAL MENTAL HEALTH NETWORK WHICH CONSISTS OF PARTNERS REPRESENTING MENTAL EHALTH, BEHAVIORAL HEALTH, PRIMARY CARE AND SOCIAL SERVICES COLLABORATING TO PROVIDE CLINICAL AND COMMUNITY-BASED SERVICES THROUGH INTEGRATED AREA COORDINATION. INNOVATIVE STRATEGIES IS THE CONTINUATION OF THE FOUNDATIONAL WORK OF THE INNOVATIVE SOLUTIONS PROJECT DESIGNED TO IMPLEMENT A COLLABORATIVE CARE TEAM MODEL UTILIZING PRACTITIONERS AND CLINICAL PROVIDERS TO SERVE AS TEAM MEMBERS TO COLLABORATIVELY ADDRESS THE CLINICAL AND SOCIAL HEALTH NEEDS OF HIGH RISK CLIENTS.
