CHESAPEAKE-POTOMAC HOME HEALTH AGENCY PROVIDES QUALITY, COST EFFECTIVE HOME HEALTH SERVICES TO THE RESIDENTS OF CALVERT, CHARLES AND ST. MARY'S COUNTIES. CHESAPEAKE-POTOMAC HOME HEALTH AGENCY STRIVES TO INCREASE PHYSICAL AND PHYSIOLOGICAL FUNCTIONING, AND SPIRITUAL WELL-BEING OF PATIENTS THROUGH HOME HEALTH SERVICES. DURING FY22 CHESAPEAKE-POTOMAC HOME HEALTH AGENCY ADMITTED OVER 2,228 PATIENTS TO CARE AND PROVIDED OVER 31,406 VISITS TO PATIENTS RESIDING IN CALVERT, CHARLES AND ST. MARY'S COUNTIES. CPHHA SPECIALIZES IN THE CARE OF GERIATRIC PATIENTS WITH OVER 81% OF OUR PATIENTS BEING OVER 60 YEARS OF AGE. OUR AVERAGE DAILY ACTIVE CENSUS IS 233 PATIENTS.
