HOMECARE PROGRAM: HOME HEALTH AIDES OR CERTIFIED NURSING ASSISTANTS PROVIDE DIRECT HOME CARE SERVICES, OR PERSONAL CARE, TO FRAIL OLDER ADULTS UNDER THE SUPERVISION OF A REGISTERED NURSE. THESE INCLUDE ASSISTANCE WITH THE ACTIVITIES OF DAILY LIVING, SUCH AS BATHING, DRESSING, EATING, OR PERSONAL HYGIENE, AND ASSISTANCE IN PHYSICAL TRANSFER, AMBULATION, AND MEDICATION REMINDERS.
NURSING PROGRAM: REGISTERED NURSES PROVIDE COMPREHENSIVE SKILLED NURSING SERVICES TAILORED TO THE PATIENTS SPECIFIC NEEDS AND SUPERVISORY VISITS TO THE CAREGIVERS.
