TRINITY AT HOME, INC. IS COMPRISED OF THREE AREAS: FAMILY CAREGIVER SUPPORT PROGRAM (FCSP), GERIATRIC CARE MANAGEMENT, AND HOME CARE. THE FCSP, A GOVERNMENT-FUNDED PROGRAM FOR FAMILY CAREGIVERS, IS MANAGED IN ROWAN COUNTY BY TRINITY AT HOME. THE PROGRAM PROVIDES RESPITE, EDUCATIONAL SUPPORT, RESOURCES, AND INFORMATION AND ASSISTANCE TO FAMILY MEMBERS CARING FOR FRAIL ELDERLY OR DISABLED ADULT WHO RESIDE IN ROWAN COUNTY. PLEASE SEE SCHEDULE O FOR A CONTINUATION OF PROGRAM SERVICE ACCOMPLISHMENTS.GERIATRIC CARE MANAGEMENT IS PROVIDED BY CERTIFIED CARE MANAGERS WHO WORK WITH FAMILIES IN ROWAN AND SURROUNDING COUNTIES IN BOTH EMERGENCY SITUATIONS AND LONG-RANGE PLANNING TO ASSIST THEM IN NAVIGATING THE SENIOR CARE SYSTEM. HOME CARE WILL BE PROVIDED ON THE CAMPUS OF TRINITY OAKS CCRC AND TO ROWAN AND SURROUNDING COUNTIES.
