IN 2021, UCARE FOUNDATION APPROVED AND DISBURSED $1,394,000 IN GRANT FUNDING TO ELEVEN MINNESOTA ORGANIZATIONS WORKING TO IMPROVE HEALTH THROUGH INNOVATIVE SERVICES, EDUCATION, COMMUNITY OUTREACH AND SUPPORTING COVID RELIEF EFFORTS WHICH ALSO HAD CORE VALUES OF DIVERSITY, EQUITY, AND INCLUSION. AN ADDITIONAL $1,000,000 WAS DISTRIBUTED IN GRANTS PROVIDED BY THE FOUNDATION USING FUNDS CONTRIBUTED BY UCARE TO SELECTED MINNESOTA COUNTIES, TRIBES, AND COLLABORATIONS OR OTHER NON-PROFITS ADVANCING LOCAL PUBLIC HEALTH PRIORITIES FOR PROJECTS ADDRESSING SPECIFIC TOPICS LISTED BELOW. - ANTI-RACISM AND HEALTH EQUITY FOCUSED WORK ADDRESSING STRUCTURAL RACISM AND SOCIAL DRIVERS OF HEALTH- DENTAL ACCESS AND ORAL HEALTH- RURAL SPECIFIC NEEDS FOR EXAMPLE, TRANSPORTATION, HEALTHCARE WORKFORCE DEVELOPMENT, AND LOCAL PUBLIC HEALTH PRIORITIES- MATERNAL CHILD HEALTH PARTICULARLY APPROACHES THAT ADDRESS INEQUITIES- INDIGENOUS HEALTH- MENTAL HEALTH, MENTAL WELLBEING, AND SUBSTANCE USE DISORDER PREVENTION AND TREATMENTUCARE FOUNDATION HAS A FORMAL BOARD THAT OVERSEES AND APPROVES ANNUAL GRANT AWARDS. THE BOARD MEETS ANNUALLY TO DETERMINE PRIORITY AREAS OF FOCUS FOR CURRENT YEAR SOLICITATIONS. GRANTEES APPLY VIA AN ONLINE GRANT SYSTEM AND ALL ORGANIZATIONS MUST MEET FUNDING ELIGIBILITY GUIDELINES. GRANT APPLICATIONS ARE REVIEWED BY UCARE MINNESOTA-ASSIGNED INDIVIDUALS AS WELL AS CERTAIN MEMBERS OF THE UCARE FOUNDATION BOARD. ONCE APPROVED, GRANTEES SIGN A CONDITIONS OF GRANT AGREEMENT AND ARE RESPONSIBLE TO SUBMIT AN INTERIM AND FINAL REPORT AS WELL AS PARTICIPATE IN A SITE VISIT BY UCARE STAFF. THE BOARD OF DIRECTORS RECEIVES AN ANNUAL REPORT ON THE USE OF THE FUNDS.UCARE HAS SPONSORED AND SUPPORTED LUTHERAN SOCIAL SERVICE SENIOR COMPANION PROGRAM FOR OVER 15 YEARS. THE PROGRAM IS AVAILABLE ACROSS MINNESOTA AND ASSISTS OLDER ADULTS, SO THEY CAN REMAIN HEALTHY AND ACTIVE IN THEIR OWN HOMESFOR AS LONG AS POSSIBLE. COMPANIONS ARE TRAINED VOLUNTEERS WHO VISIT THREE TO FOUR HOURS PER WEEK TO OFFER SUPPORT, FRIENDSHIP, AND TRANSPORTATION IN THE COMMUNITY. THE PROGRAM ADDRESSES MEMBER ISOLATION AND LONELINESS AS WELL AS ALLOWS ELDERS TO REMAIN LIVING IN THEIR HOMES INDEPENDENTLY. HEALTH OUTCOMES ARE TRACKED WITH THE GOAL TO SEE ENHANCED QUALITY OF LIFE AND HEALTHIER LIVING FOR THOSE RECEIVING SERVICE.
