SENIOR HEALTH430 CLIENTS SERVED IN THE PROGRAMS IN 2023. OF THOSE 430 CLIENTS,67% REPORT MEDICATION SELF MANAGEMENT AFTER WORKING WITH NURSE. 100% RECEIVED CHRONIC DISEASE EDUCATION. 10% HAD A FALL WHICH DID NOT RESULT IN INJURY.
