PACE (PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY) PROVIDES AN IDEAL ALTERNATIVE TO NURSING HOME PLACEMENT. PACE ALLOWS INDIVIDUALS IN NEED OF SKILLED NURSING CARE TO REMAIN IN THEIR HOMES OR COMMUNITY SETTING AND RECEIVE QUALITY CARE FROM A TEAM OF CLINICAL EXPERTS. WE PROVIDE CUSTOM- TAILORED SERVICES INCLUDING NUTRITION SUPPORT, IN-HOME PERSONAL CARE, THERAPY, PSYCHIATRIC SERVICES, AND TRANSPORTATION TO ENSURE THAT EACH PARTICIPANT IS HOLISTICALLY SUPPORTED ACCORDING TO THEIR NEEDS.
