FAMILY-FRIENDLY PRIMARY CARE PRACTICES ARE ALL PATIENT CENTERED MEDICAL HOMES AND UNIQUELY DEDICATED TO PROVIDING HIGH-QUALITY, ACCESSIBLE CARE.
